Musical Performance Application
All applications must be accompanied by a $10.00 application fee ($5.00 if application
is completed online), a completed stage setup sheet, and a demo CD/DVD. NO CASSETTE
TAPES PLEASE! No application will be considered complete unless the above mentioned
items are submitted by the Application Deadline of February 29, 2008.
Important Note: The Musical Performance applications are open to residents of the following West Michigan counties: Allegan, Barry,
Ionia, Kent, Montcalm, Muskegon, Newaygo, Ottawa. If the mailing address provided
below is not in one of these counties, please describe your connection to the area
in the "Additional Information" field at the bottom of the application.
